Cipriani, on hearing rumours about Casa's activities, decides to follow him. Toni Cipriani goes to visit his mother at her restaurant after four years but, upon his arrival, is told he does not measure up to deli owner Giovanni Casa. Joseph Daniel O'Toole tells Toni Cipriani of the Sindacco run The 'Dolls' House, a casino/bordello in the Red Light District, and suggests that Cipriani destroy it to weaken the Sindacco Family. Cipriani then sets about protecting the casino. Joseph Daniel O'Toole tells Toni Cipriani that the Sindacco Family are planning to attack the Leone Casino in Saint Mark's using car bombs. Joseph Daniel O'Toole tells Toni Cipriani that the police are about to raid Paulie's Revue Bar and that he needs to get Salvatore Leone away from the club and police. Toni Cipriani wants Joseph Daniel O'Toole to pay him his money, so O'Toole and Cipriani travel around the Red Light District to collect money from the O'Toole's prostitutes.

Vincenzo Cilli tells Toni Cipriani that he wants him to pick up a car full of drugs from just outside Portland Harbor. Vincenzo Cilli sends Toni Cipriani to help some of his men who have messed up a raid on a AmCo petrol station in Harwood. Vincenzo Cilli sends Toni Cipriani to meet Sindacco Family associate Joseph Daniel O'Toole, the manager of Paulie's Revue Bar, who wants to join the Leone Family. Vincenzo Cilli wants Toni Cipriani to kill a number of Sindacco Family mobsters in Chinatown after the drug dealer Toni had previously met was killed.

Vincenzo Cilli has Toni Cipriani locate a drug dealer and take the reluctant man to his patch in Chinatown. Toni Cipriani drives Vincenzo Cilli first to his new safehouse in Saint Mark's, where he changes clothes, and then drives Cilli to his warehouse in Atlantic Quays. He then arrives at Salvatore Leone's mansion where he is told he will be working for Vincenzo Cilli. Toni Cipriani returns to Liberty City at the Punk Noodles store in Chinatown, Portland, before using a public phone.
